Spotlight on success: Hospital Transformation Program (HTP) milestones

The goal of the Hospital Transformation Program (HTP) is to improve the quality of hospital care provided to Health First Colorado members.

The Hospital Transformation Program (HTP) is a five-year initiative aimed at enhancing the quality of care for Health First Colorado (Medicaid) members by linking hospital payments to performance-based outcomes. Now in program year four, HTP continues to drive improvements in patient care, community engagement, and health outcomes across Colorado.

Celebrating program year three achievements

All six HCA HealthONE hospitals were recognized as top performers in the state for their collaborative discharge planning for patients with mental illness or substance use disorders. HCA HealthONE Sky Ridge also stood out for its leadership in reducing opioid use in emergency departments through the Alternatives to Opioids (ALTO) initiative.

Hospital highlights

  • HCA HealthONE Mountain Ridge: AnnMarie Estes and the emergency department (ED) and patient access teams were praised for their dedication to screenings and transitions of care.
  • HCA HealthONE Aurora: Lea Mooney brought creativity to HTP celebrations, while the telehealth, case management (CM), and ED teams earned accolades for innovation and resilience.
  • HCA HealthONE Rose: Juanita Thomas, Diana Bennett, and Brad Basham led in screenings and stroke care, with strong team efforts in depression screening.
  • HCA HealthONE Sky Ridge: Brittany Stubblefield and the ED team excelled in depression screening and ALTO performance.
  • HCA HealthONE Presbyterian St. Luke’s: Lily Gooch and the ED/case management teams led the division in Screening, Brief Intervention and Referral to Treatment (SBIRT) performance.
  • HCA HealthONE Swedish: Diane Kennedy and the case management/social work teams improved follow-up care and depression screening rates.
